Participant Agreement, Release, and Acknowledgment of Risk

This Health and Safety Waiver is executed on the scheduled appointment for contrast bathing by the undersigned individual, also named in the booking, in favour of Other Fold Sauna Co.

By signing this Waiver, I voluntarily agree to use the sauna facilities provided by Other Fold Sauna Co. under the following terms and conditions:

1. Health Acknowledgment

I affirm and acknowledge that:

1.1. I am in good physical health and have no medical condition or health concern that would prevent me from safely using the sauna.

1.2. I have consulted with my physician if I have any pre-existing medical conditions, including but not limited to:

• Cardiovascular conditions (e.g., heart disease, high blood pressure).

• Respiratory issues.

• Pregnancy.

• Diabetes or other metabolic disorders.

• Any other condition that may affect my ability to safely use the sauna.

1.3. I am not under the influence of drugs, alcohol, or any substances that could impair my judgment or physical abilities while using the sauna.

2. Risks of Sauna Use

I understand that sauna use involves potential risks, including but not limited to:

• Dehydration or heat exhaustion.

• Heat-related illnesses such as hyperthermia.

• Dizziness, nausea, or fainting.

• Burns from prolonged contact with heated surfaces.

I voluntarily assume all risks associated with sauna use and agree that I will follow all posted rules, safety guidelines, and instructions provided by Other Fold Sauna Co. or its staff.

3. Release of Liability

I hereby release, waive, and discharge Other Fold Sauna Co., its officers, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action related to:

3.1. Injuries, damages, or losses sustained during or as a result of sauna use.

3.2. Any negligence on the part of Other Fold Sauna Co. or its staff.

4. Indemnification

I agree to indemnify and hold Other Fold Sauna Co. harmless from any claims, damages, or expenses arising out of my use of the sauna, including attorney fees and court costs.

5. Rules and Guidelines

I acknowledge that I have read, understand, and will comply with the following:

5.1. Limit sauna sessions to the recommended duration (typically 15–20 minutes).

5.2. Stay hydrated by drinking water before and after sauna use.

5.3. Refrain from using the sauna if I experience any discomfort or adverse symptoms.

5.4. Avoid bringing glass, sharp objects, or flammable materials into the sauna.

5.5. Follow all instructions provided by Other Fold Sauna Co. or its representatives.

6. Age and Capacity

I affirm that I am at least 18 years old (or have obtained parental/guardian consent if under 18) and have the legal capacity to sign this Waiver.

7. Binding Agreement

This Waiver is binding upon me and my heirs, assigns, personal representatives, and next of kin.

8. Governing Law

This Waiver shall be governed by the laws of the United Kingdom], without regard to its conflict of laws principles.

Acknowledgment and Signature

By signing below, I acknowledge that I have read and understand this Waiver, and I voluntarily agree to its terms. I understand that this Waiver limits my legal rights and remedies and that I am signing it freely and voluntarily.
